Referrals were reaching three treatment centers in Ohio and Florida with no central record of where they came from or what happened to them. Tracking depended on spreadsheets and memory, and leadership couldn't see which sources sent patients, why referrals were denied, or where they stalled.
The problem
What we built
- A phone and web app where staff log every referral and its source in under a minute.
- Each referral goes to the right facility's intake team, and the first person to claim it owns it.
- Every referral moves through clear stages, from pre-assessment and insurance check to admitted, and every denial needs a reason.
- If one facility is full, the same referral moves to another location without being re-entered.
- Inquiries from the Ebb Tide, Alora and Novus websites land in the tracker on their own.
- Ohio and Florida each see their own referrals; ownership sees everything combined.
What it solved
- Every referral has an owner and a status, so none slip through the cracks.
- Leadership sees which referral sources actually turn into admissions, and why others don't.
- Referrals nobody has touched get flagged after 24 hours.
- Website inquiries become referrals with no retyping.
